We are looking for a dynamic and passionate Registered Nurse--Transitional Care Unit to join the Mountainside Team!

What You’ll Do

The Registered Nurse--Transitional Care Unit provides each patient with routine daily nursing care, including administering medications and performing treatments and procedures, and other desired services in accordance with the patient’s assessment, choices and care plan.

The successful candidate will be able to:

  • Provide and coordinate direct care in accordance with the principles of patient and family-centered care based on the nursing process and according to the clinical standards of the hospital

  • Identify and communicate individualized patient needs for safe handoffs during transitions of care

  • Formulate a goal directed plan of care which is prioritized and based on determined nursing diagnosis and patient outcomes

  • Implement care in a safe, knowledgeable, skillful, consistent and continuous manner

  • Participate in patient teaching and discharge planning

  • Seek supervision, consultation, assistance when patient is unable to perform safely and independently

  • Work closely with the Nursing Supervisor and the interdisciplinary team to manage the day-to-day functions

  • Perform any and all other duties assigned

What You’ll Bring

  • Associates’ Degree in Nursing required; Bachelor’s Degree in Nursing (BSN) preferred
  • 1 or more years of nursing experience in acute, subacute and long term care required
  • BLS certification required
