Clinical Coordinator

Shift: Day
Schedule: Full - Time Regular

Essential Functions:

  • The Clinical Coordinator is expected to maintain a high degree of professional and ethical competence while serving as part of the multidisciplinary care-giving team
  •   Under the supervision of the Clinical Director or Practice Administrator, Supervises and serves as a role model for Licensed Practical Nurses and Medical Assistant staff while ensuring continuous provider support
  • Continuously monitors the EMR patient tracker and patient screening to prevent delays in provider scheduled appointments and acts to ensure constant flow threw out the site
  • Collaborates with providers to accommodate scheduling late, urgent and same-day visits
  • Responsible for receiving and responding to all calls from the call center and handle them accordingly  
  • Performs/monitors vital signs obtained by LPN/MA staff, screen, interview, counsel and provide direct patient care. Document all clinical observations, findings, and services provided in the patient’s electronic medical record (EMR).  Report all abnormal observations and findings to the appropriate medical provider in a timely manner verbally or via skype
  • Performs/monitors clinical staff’ s documentation in the EMR of all ACO / CPC+ measures to ensure capture of all required fields including patient education on health maintenance, preventive health, and medication adherence  
  • Performs/monitors clinical staff assigned Rx, Results and Office voicemail folders daily, and the Nurse/ physician pools daily for tasks, follow-ups as assigned, send Urgent Lab Recall and Missed Appointment letters, as required and acts to prevent back up and delays in patient/ provider needs  
  • Performs/monitors LPN/MA staff preparation and documentation of patient materials for various procedures/treatments (e.g. Medical/Surgical Clearance, pap smears, biopsies, STD screening, colposcopy, I.&D, etc.) including consents and patient educational material  
  • Performs/monitors and documents the following tests/procedures/treatments as ordered by providers including but not limited to:  EKGs, CLIA waived Testing, Venipuncture /obtaining Labs Specimens and Administering Vaccines according to Center protocol. Assist with examinations and procedures (i.e. colposcopy, pelvic exam, urinary catheterization, endometrial biopsies, LEEP, IUD insertions, etc.), as needed and as per national standards and Center protocols
  • Performs/monitors LPN/MA staff reporting, documentation, and follow up on patient laboratory/diagnostic tests results as instructed by medical providers accordingly while maintaining patient confidentiality policy  

  • Maintains medication inventory and orders medical equipment/supplies as needed and acts on equipment malfunctions for repair  
  • Creates and monitors monthly staffing schedule and daily assignments to identify, plan and reassign/ float clinical staff as needed in collaboration with fellow Clinical Coordinators to maximize staff utilization and promote efficient provider support and patient care  
  • Responsible for following/ promoting network quality initiatives and national standards with staff compliance in infection control guidelines, i.e. hand hygiene, cleaning, disinfecting, sterilization and maintaining clinical equipment and medical instruments as per center’s protocol  
  • Monitors and order clinical supplies to maintain a baseline PAR level of medical supplies and equipment in all clinical areas  
  • Maintains refrigerator, freezer and ambient room temperature logs and CLIA waived test logs daily and acts on out of range temperatures per protocol  
  • Performs, monitors, and promotes patient care in accordance with current Joint Commission National Patient Safety Goals  
  • Participates/monitors Quality Improvement activities as directed by the Clinical Director/or Office Manager (i.e. quality control measures, peer chart review, patient satisfaction surveys, etc.) to ensure the provision of quality nursing services to Hackensack Meridian Health’s Network population  

  • Assists in the onboarding and orientation process of new staffing hires  
  • Promotes professional development and clinical excellence through supervision, mentoring, and orientation of LPN/MA staff  

  • Provides on-going feedback and acts on issues regarding clinical staff’s performance and time/attendance in collaboration with the Clinical Director or Practice Administrator  
  • Reviews and manages all PTO requests of LPN/MA staff and approve based on seniority while ensuring appropriate provider coverage prior to awarding time  
  • Reviews, approves and submits time sheets to payroll for LPN/MA staff bi-weekly via API  
  • Attends health center and network meetings, as required and assigned  

  • Executes corrective action plans under the direction of the Clinical Director or Practice Administrator  
  • Performs other duties as assigned by the Clinical Director and/or other administrative personnel

Minimum Education

  • Associates, High School Diploma, or GED equivalent preferred
  • Graduate from an accredited Medical Assistant/Medical Technologist school/program and/ or school of nursing and holds a valid Certificate of Completion.  

Minimum Work Experience

  • 1-3 years of experience in ambulatory setting (emergency room, clinic or health center).
  • Higher education may be substituted for experience  

Preferred Qualifications

  • Bilingual English/Spanish or other language
  • Previous supervisory experience in an acute/outpatient setting

Required Licenses/Certifications

 

  • Maintain a current National Certification as Certified Medical Assistant or NJ LPN License
  • CPR Certification preferred

Required Skills, Knowledge, and Abilities

  • Strong organizational skills and ability to multitask and adapt to the needs and demands of a fast-paced clinical environment.
  • Working experience with electronic medical records (EMR) software.
  • Basic skills in a Microsoft environment:  MS Outlook, Word and Excel
